According to the American Lung Association (ALA) it affects between 6.7 and 9.6 million U.S. children under the age of 18, American Lung Association (ALA) (as cited in Toole 2013). Asthma is the most common chronic childhood disease with increasing prevalence from 31.4 per 1000 population in 1980 to 54.6 per 1000 population in 2000 despite the advances in asthma pathophysiology understanding and treatment (Tsakiris, Iordanidou, Paraskakis, Talskidis, Rigas, Zimeras, Katsardis, Chatzimichael, 2013). Although there have been new medications and medical advances, asthma is a significant cause of a morbidity, school absenteeism, parent lost work days, emergency department (ED) visits, and hospitalizations for children all over the world. Brown, Gallagher, Fowler, Wales; Martinez; Mattke, Martorell, Sharma, Malveaux, Lurie (as cited in Toole 2013). Looking into the causes of school absenteeism, it has been found that asthma is the most frequent cause, according to Doull et al., ââ¬Å"55% of school students and 55% of asthmatic students missed school days due to respiratory symptoms.â⬠Attendance and the limitation of daily activities are both used as indicators of asthma control level in children. Increased absenteeism interrupts learning processes and participation in daily activities. Unfortunately ââ¬Å"In a study that specifically focused on parentsââ¬â¢ report of receiving written self-management tools from pediatric primary care physicians, Cabana et al. (8) found that only about 30% of parents reported receiving these tools known to facilitate childrenââ¬â¢s medical adherence.â⬠(Orrell-Valente, Jones, Manasse, Thyne, Shenkin, Cabana (2011). An initial literature review was done to gain information about what barriers impact medication compliance with school age children. Using different keywords: children, medication compliance, asthma, cost, education, barriers and impact of nonÃâà compliance; were used in the CINAHL database, Google Search, and the Simmons Library to locate information on the subject. Further searches were conducted to refine the topic, from medication compliance with school age children to a more specific topic of medication compliance and asthmatic children. Identifying asthma as the main subject allowed for us to move in a more specific direction. Our next pursuit was in identifying and categorizing the different barriers, determining the major and minor subjects and listing them under specific categories. ââ¬Å"No one risk factor is responsible for asthma morbidity; rather a plethora of factors contribute to the high prevalence, which vary dramatically among children with asthma (Clark, Mitchell, Rand, 2009). Asthma risk factors include living in poverty in the inner-city, being uninsured or Medicaid enrolled, and being African American or Hispanic (Akinbami, Moorman, Garbe, Sondik, 2009; Bloomber et al., 2009; Gerald et al.; Liu Pearlman, 2009; Mattke et al., 2009; Smith, 2009) (Toole, 2013 p 115).â⬠ââ¬Å"In 2005, 9% of children under the age of 14 years were diagnosed with asthma and the prevalence of asthma was found to be highest in this age group (Center for Disease Control and Prevention, Control and Prevention, 2006).â⬠(Kamps, J. L., Rapoff, M. A., Roberts, M. C., Varela, R. E. Barnard, M., Olson, N., 2008 p. 206). Critiquing the research articles that were found has led to three major barriers in asthma management. The first barrier is in cost. Subcategories of cost include insurance availability, income, and socio economic levels. The second is culture. Subcategories of culture include language barriers, legal status, traditions and use of alternative medicine. The last is education. Subcategories of education include health literacy, education level, reading and comprehension abilities, information provided and follow up. Barriers that impact and interfere with the management of asthma in children are varied. The outcome of ineffective management are increase cost, hospitalizations, improper use of medication and death. Health care providers need to ensure that the patient and parent or guardian understand the proper use of medication, the disease process and associated risk for misuse of medication. Using these categories, a literature review will be a guide in determining the best practice for improving outcomes, decreasing cost, and developing a plan to ensure cooperation between parents, children and the health care provider. Asthma management requires a multi-faceted approach, including an effective educational component (Ambulatory Pediatrics, 2006). Poor patient outcomes have been associated with a lack of patient and parent compliance with the patientââ¬â¢s individualized treatment plan. There are a number of possible factors that may play a role in patientsââ¬â¢ and parentsââ¬â¢ noncompliance. They include financial and cultural barriers, and parentsââ¬â¢ and patientsââ¬â¢ misconception about the disease process and the importance of treatment (Cleveland, 2013). The trends reported in a recent study indicated that asthma education to parents positively impacts asthma-related outcomes in children (Kielb, Lin, Hwang, 2007). In this small sample, there was a decrease in asthma-related sick visits post-education. Asthma cost are increasing and responsible for a higher percentage of the total health care cost for treatment. Increasing and changing copayment are leading to more emergency room visits and hospitalizations. The cost of these are not as visible as the direct cost of an inhaler medications. So the need for educating on all the cost of asthma are important. In the article ââ¬Å"Outpatient Management of Asthma in Childrenâ⬠by Andre Schultz and Andrew C. Martin, they discuss the roles of the provider in the diagnosis and treatment of asthma in children. This article determined that one of the critical areas is non adherence to treatment. Having a plan in place is important as well as continued follow up, avoidance of triggers, and use of medication. Non adherence to medication is impacted by the several factors. Socioeconomic status plays a large role in adherence to medication. Data obtained shows that lower adherence is reported in children at a higher rate from low income families. Perception of cost and the discussion between the Practitioner and patients is important. Determining how the client feels about the medication, treatment plan and chronic disease is important. This will help to facilitate the response to care. The perception of the cost of medication on the client will play a significant role. In a quasi-experimental study done in 2010, they compared participants in a control and intervention group in regards to adherence to medication, healthcare cost and resource utilization. The determined intervention consisted of 2 components. One an average reduction in copayment and the second was mailing educational material for asthma management. Adherence was determine by the medication available during the duration of therapy and total supply of medication divided by the duration of therapy. When refills overlapped, it was assumed that the client consumed all medications. Healthcare resource was determined by office visits, hospitalizations, emergency room visit, short acting beta-agonist canisters and oral corticosteroid prescriptions. Cost were defined as total amount paid for visits, hospitalizations, emergency room visits, and prescription drugs. Overall cost were determined during the twelve month follow up period. Monthly cost were used rather than total cost during the st udy period. This study showed improved adherence to controller medication which translated into reduced medical cost and increased prescription cost. Although there were an increase in prescription cost the overall expenditure decreased. This study determined that increasing copayments will create a financial barrier to medication adherence. In children, parents are the main administers of medication. A link between the socioeconomics, cultural values, education and use of medication has been shown to produce a negative effect on adherence. This effect is not a single factor but many factors grouped together to provide a complete picture. The Kite Runner Essay -- Literary Analysis, Khaled Hosseini
As implied by the title, kites play a major role in the novel The Kite Runner by Khaled Hosseini. They appear numerous times within the text and prove to be surprisingly versatile in their literary function. They provide common ground for characters whose interests do not normally intersect. They are also present as a very powerful symbol, which adds an extra dimension to this already literary rich novel. Reversing the roles transcending generations, it shows itself to be a multifaceted medium. This novel presents two almost irreconcilable individuals. The main character, Amir, was raised without a mother; therefore, Amir was left with his father, Baba, to please as he grew up. Early on in his life, it became obvious that pleasing Baba would prove to be problematic. They simply did not have similar interests as a consequence of Baba ââ¬Å"fathering a son who preferred burying his face in poetry books to huntingâ⬠(Hosseini 19). Baba was described as a very dominating figuring standing at a monstrous six feet five inches tall who often enjoyed hunting and ran his own business. Amir, on the other hand, was of insignificant stature even for his young age and often was found reading poetry and stories with his friend and servant, Hassan, who, although being a year younger than Amir, often beats Amir in the area of athletic prowess such as throwing rocks when ââ¬Å"Hassan made his stone skip eight times. The most I [Amir] managed was five,â⬠(Hosseini 14) or even si mply running when Amir said, ââ¬Å"Hassan ran faster than I [Amir] did, and I was falling behind,â⬠(Hosseini 53). The son was simply not very sportive. Inversely, The athletic father also greatly enjoyed playing soccer as a child and later enjoyed being a spectator of this sport; consequ... ...r image had occurred from the past with the reversal it entails. The kite is important in this situation because the reversal could not have taken place in any other scenario. Sohrab would not interact with the world nearly at all at this point, and the only way it could have paralleled would be with a kite battle. Amir had traded places with Hassan through Sohrab with the use of the kite. Wholly proving to be a very dynamic object in this novel, it functions on multiple levels. It provides a connection between two characters that reside in totally different spheres of interests. It acts on the symbolic level as a symbol of freedom for the characters within. It also provides a medium for the reversal of roles between two characters in this novel. Kites prove to be quite potent in their presence and function in the novel, The Kite Runner by Khalid Hosseini.

Background to Project Over the years, the use of information communication technology has had a dramatic impact on almost all areas of human operation including, in particular, the banking and finance sector. To a certain extent, ICT can be seen to have simplified many of the business processes; however, it has also created a new breed of criminal activity, many aspects of which are aimed at the banking sector, for example, credit card fraud, identity theft and ATM fraud, as well as other related information technology issues (Singhal & Padhmanabhan, (2008). The proliferation of e-banking has offered opportunities in countries such as Nigeria to become much more global in its operation and yet it has also opened up the jurisdiction to a broad range of cyber-crimes, which will be the focus of the analysis in this research. E-banking is not limited to the more developed countries and, arguably, can be seen to transcend many of the jurisdictional borders, with countries which are developing, such as Nigeria, being able to trade internationally in a way that was not possible, previously. However, this opportunity also presents challenges, particularly when it comes to establishing systems that will prevent or limit cyber-crime. Problem Specification E-banking in Nigeria is still largely seen to be in its infancy, with the majority of banks using information technology as a means of providing information, rather than creating an interactive banking arrangement (Howard et al 2008). To a certain extent, it could be argued that this relatively low level of take-up for e-banking in Nigeria may be attributed to the insecurity and the failure of the technology to keep pace with the needs of the public, if they are to fully engage with internet banking and all its ramifications (Chiemeke, Evwiekpaefe, & Chete, 2006). Crime and corruption within the banking sectors have created a particular concern for businesses across Nigeria, which indicates that dealing with the area of cyber-crime is critically important and needs to be tackled, if Nigeria is to be able to develop as a strong international economic jurisdiction capable of attracting investment and stability. Research Aims and Objectives The overall aim of this research is to look at the evolution of cyber-crime in Nigeria and to explore ways in which it can potentially be managed, or even eradicated. In order to achieve this over-riding aim, several smaller research objectives have been identified. Firstly, it is necessary to gain an historical understanding of the Nigerian banking industry and to ascertain the ways in which the industry has operated, prior to the use of information technology, and how information technology has impacted upon the banking sector, to date. Secondly, an analysis of the current cyber-crime control processes will be undertaken, looking at a range of different banks and how they look towards managing and controlling cyber-crime (Computer Crime Research Center 2009). The research will then move on to compare banks in other regions, such as the United Kingdom and the United States, with a view to establishing whether there are additional ways in which cyber-crime could be managed more efficiently, by looking at the various different means whereby cyber-crime is managed in these other jurisdictions. The findings can then be looked at in the context of the problems being faced in Nigeria, and potential options explored. A specific case study of GTB will be used, in order to explore the issues raised above, before ultimately suggesting how the Nigerian banking sector can improve its position and reduce the level of cyber-crime, to such an extent that it is likely to improve the stability and trust within the banking sector. GTB in particular has been the subject of much discussion due to several high profile fraudulent activities that have raised the issue of managing cybercrime in Nigeria (Shittu, 2010). Offering this guidance is a critical element of the research as it will offer guidance on how to manage the difficulties being raised by the Nigerian banking industry. Research Methodology The research approach will be inductive in nature in that it will look at a wide variety of ideas and observations with the view to ultimately establishing a theory and a framework that can be relied upon to develop the future of the Nigerian banking industry. In order to achieve this, both quantitative and qualitative analysis will be undertaken. However, this will primarily involve looking at the data associated with cyber-crime, in the region, as part of the secondary research and then going on to undertake a case study, to improve the understanding of where the data has originated from, and in order to bridge between the theory and practical reality of cyber-crime, in the region. Resources Resources will primarily be derived from journals and reports dealing with the area of cyber-crime, in Nigeria, although it will also be necessary to undertake primary research with the employees of the case study bank, i.e. GTB. Suggested references to be used as a starting point are listed at the end of the proposal. Constraints Potential issues may emerge when it comes to collecting accurate data, as issues related to cyber-crime rates can be commercially sensitive, thus making it harder to ensure that the data and the subsequent analysis are accurate. It is also necessary to recognise that issues relating to cyber-crime are likely to change, very rapidly, making the research quite time-specific, in terms of its value. Gaining detailed primary research by interviewing employees may also present a problem, as employees are unlikely to be prepared to speak openly, for fear of any comeback from their employer. Projected Schedule The research will take place over one academic year, which actually spans a total of nine months.
