Critical Analysis of Pharm.D in India Essay
SUMMARY Critical Analysis of Pharm.D. in India: Exploring the Way Ahead INTRODUCTION: Pharmacy education in India was mainly focused on pharmaceutical science courses rather than clinical/pharmacotherapeutic courses until 2008, when Pharm.D. was introduced and began to be offered in India, to focus towards clinical and community aspects. The study was conducted to analyse the current scenario of Pharm.D. in India and look at possible issues & options. OBJECTIVE: The main objective of the study is to find out the current status of Pharm.D. from studentsââ¬â¢ perspective and to identify the need of Pharm.D. as a part of Pharmacy Education System in India. Addressing the possible shortcomings and give probable suggestions to improve the scope of the same. EXPERIMENTAL METHOD: The study comprises of an extensive secondary research and also a primary research to understand the current scenario of Pharm.D. from studentsââ¬â¢ point of view. A literature survey was done to study the introduction and status of Pharm.D. curriculum in India and the achievement of Pharm.D. in western countries. Various journals and publications from the database engines of Ebsco, Emerald & Google Scholar were studied and referred to analyse the evolution and future of Pharm.D. Also, a primary research was conducted in Manipal College of Pharmaceutical Sciences, Manipal University, Karnataka. The selection of survey location (MCOPS) was done on the basis of a combined list from ââ¬ËIndiaââ¬â¢s Best Pharma Collegesââ¬â¢ (Careers 360 July 2011 Edition) and ââ¬ËInstitutes offering Pharm.D. and Pharm.D. (Post Baccalaureate) Coursesââ¬â¢ (Published by Pharmacy Council of India). A questionnaire was made on a 5 point Likert Scale and circulated online amongst the students of MCOPS. The responses were analysed by using the online softwares of Survey Monkey survey portal. RESULTS AND DISCUSSIONS: Out of 130 respondents to the survey, a total of 109(83.83%) respondents said that the curriculum of Pharm.D in India is justifiable for healthcare sector and 97 (74.61%) said that the internship of Pharm.D students meets the requirements of the hospitals/Clinical Research Organisation. 24 responses (18.46%) opposed the opinion of providing the prescribing rights to the Pharm.D. graduates and 69(37.69%) voiced that such a right, if provided, will not be accepted by the doctor community. Figure 1 is indicative of the above stated data. FIGURE 1 Closed ended questions were put to investigate the probable reason(s) for Pharm.D. lagging in India from the west. 115 responders (n=130) said that the Pharm.D. graduates will not be accepted by the Doctorsââ¬â¢ Community. 112 responders said that the exposure to clinical pharmacy was less and 117 were pessimistic about the awareness on pharmacy profession in India. The interpretation of the survey results shows that one of the probable reasons of selection of Pharm.D. course by students was the alluring tag of a ââ¬ËDoctorââ¬â¢. However, the critical analyses lead to a consensus of lack of required clinical exposure and knowledge of the field of diagnosis and therapeutics. If the Pharm.D. program can incorporate a few alterations in the curriculum and develop a practice-based academic unit bridging the pharmacy practice with academia, there is great hope for the Pharm.D. program in India. CONCLUSION: The findings of the survey revealed that the exposure of the Pharm.D. students to the clinical and diagnostic aspect of the therapeutic system needs to be increased. Plata o Plomo (Javdani) Response Paper Analysis Essay
In her essay, ââ¬Å"Plato o Plomo,â⬠Marie Javdani compares & contrasts the stories of two boys, whom live separately in two countries, to signify their indirect relation. The phrase ââ¬Å"Plato o Plomoâ⬠translates to ââ¬Å"silver or leadâ⬠, meaning that peasants of South America can either accept a bribe & live (silver) or take a bullet & die (lead). Eric, an American boy, & Miguel, a Colombian, are both out on the road on a Friday night, but their intentions are entirely different. After scoring drugs, Eric whistles while walking down his street to meet up with his friends for ââ¬Å"a bit of fun.â⬠Miguel creeps down the road in his village, praying for the last time in his life; he to be murdered by the guerillas who have been threatening him & his father. The two stories of both Eric & Miguel begin to unite as Javdani narrates the cause & effect of America & Colombia, showing how choices made in the U.S. can affect the harsh realities of Colombiaââ¬â¢s drug cartel. By using parallelism in the stories of Eric & Miguel, Javdani identifies the factors contributing to high drug activity. ââ¬Å"Eric & Miguel represent opposite poles in what the United States government refers to as the ââ¬Ëwar on drugs.ââ¬â¢Ã¢â¬ Ericââ¬â¢s drug use symbolizes the demand of production in Colombia, where Miguelââ¬â¢s village is terrorized by the ruling drug lords & paramilitaries. The strategy that the writer uses to represent both Eric & Miguel is cause & effect. This use of parallelism combined with cause & effect is meant to emphasize two sides to the political turmoil of drug violence in Colombia. Javdani mentions that U.S. money being sent to Colombia is ineffective in its purpose, which is to enforce order over the high drug activity that rules Colombia. Because Colombia produces a majority of the worldââ¬â¢s cocaine & heroine, the U.S believes that putting an end to the growth of the coca, a plant used for making cocaine & heroine, can stop the use of drugs in the states. But the billions of aid dollars sent to fund, supply, and train Colombian military units have only escalated the violent paramilitaries that support drug cartels. As a result of rebel drug lords having control, Colombians have no choice but to cooperate with the production of coca on their land. This approach to eliminating drug activity was obviously not successful, it is becoming easier to see that drug trafficking is market-driven; the end to the demand.

On the other hand, the divorce from the bond of matrimony is the absolute form of divorce, where the links between a husband and wife are permanently se vered. Annulment is the other type of divorce, which takes the form of a legal decree that declares a marriage null and void, on the ground that such a marriage was entered under conditions of fraud, duress or coercion of one of the party (Stim, 2013). The Grounds for Divorce in the State of Virginia Under the Family Laws of the State of Virginia, it is provided that a divorce cannot just occur out of the mere agreement between the husband and wife. Under this law, it is required that sufficient ground should exist for effecting a divorce between a husband and wife, even where the two have mutually agreed to have a divorce (Gross & Callahan, 2006). The bases for the two types of divorces are provided for differently. The required grounds for completing a divorce from bed and board, as provided for in the State of Virginia, include a Willful desertion or abandonment, where the individuals involved have broken the bonds of cohabitation, and they have the intent to dessert or abandon e ach other (Virginia State Bar, 2013). The speakers context here is the basic comic devices of reversal and exaggeration. When the dimensions of things are reversed there is a comic effect. When clowns at the circus ride around in a tiny car the effect is hilarious. In a famous Gary Larsen cartoon a gigantic monster is seen peering into a mans car through the wing mirror which reads: Things reflected in this mirror may appear to be larger than they are. The comic context employed by the speaker in the following passage, then, is that of a man suddenly turned tiny by circumstances beyond his control. There are, of course, classical antecedents for this type of size reversal. Odysseus in the cave of Cyclops would provide the best example. There are, no doubt, many who would argue that this incident in the Odyssey is not meant as humor. May we not at least wonder, however, if some of Homers audiences didnt chuckle when they heard about how the subtle Odysseus outwitted the giant? It will be argued in the following that Swifts intention throughout Part II as a whole is comic irony, and that the passage to be analyzed typifies the situation in which Gulliver finds himself when surrounded by giants. Starting off, a simple exaggeration introduces the passage: The Kings palace is about seven miles round suggesting the colossal size of the castle, the rooms within are two hundred and forty Foot high. Gulliver who is, as we have learned earlier, a proud and dignified man is reduced by his comparatively tiny dimensions to the role of a doll. All of his proud bearing and gentlemanly dignity disappears in a puff of smoke when his Mistress Glumdalclitch holds Gulliver up in her hand to give him a better view of the surroundings. Swifts choice of words at the beginning of this passage also provides an ironic effect. Gulliver who is, in fact, a freak in this society reports that when Glumdalclitch is taken out to see the town, I was always of the party, carried in my Box To be of the party suggests social (and physical) equality, but when Swift follows this dignified phrase with the description in my box the effect is humorous, since Gulliver is revealed as the curiosity and freak that he is by the fact that he travels in a box like a doll. Swifts imagery in this passage allowed allows the reader to see other human-like creature from the perspective of a very tiny person. It also demonstrates to the reader once again that Swift loves to engage in the humor of the disgusting and the impolite. When a group of Brobdingnagian beggars presses up against the carriage to view the strange little creature that is our speaker, Gulliver is able to observes the cancer on the breast of a beggar woman full of holes, in two or three of which I could have easily crept and body lice and their snouts with which they rooted like Swine. There is a misogynist quality to this joke. The breast of a woman is presented as disgusting rather than as an inspiration to art and poetry. The idea of crawling into a cancerous lesion on a womans breast is an ugly parody of what men usually think about when they see the naked female breast which is to adore, kiss, or suck it. This type of humor is based on a simple reversal of the usual emotions inspired by an image. The equivalent would be, for example, to provide an image of the Queen of England sitting on a chamber pot rather than her throne. The imagery in the rest of this passage is also unforgettable, especially the wooden legs of a beggar which were each about twenty Foot high. Immediately following these alarming and disgusting images is another liars trick based on the category of emphasis. This is offered in Gullivers careful description of his Box. Before analyzing this part of the passage in detail a general comment on Swifts project in Gullivers Travels is required. The speaker mentions many times throughout the tale the phenomenon of travelers tails or books of voyages. These were supposedly factual accounts of what travelers from Europe had seen on the other side of the world. They were, of course, full of lies and Swifts project throughout much of the book is to satirize the lying authors of these books. One well known liars trick is to emphasize the details of some fictional object. This is what Gulliver does with the description of his Box. Its origin is carefully described: the Queen ordered a smaller one to be made for me Its design and dimensions are carefully recorded: This traveling Closet was an exact Square with a Window in the Middle of three of the Squares , etc. The important detail of the boxs construction which will eventually allow for Gullivers salvation by sailors is also carefully noted: On the fourth side, which had no windows, two strong staples were fixed , and so on. There is a dual purpose to what we might call the liars emphasis lavished on this passage. The first is to satirize the books of travelers tales so popular in Swifts days in which exact descriptions of fantastic creatures were given to fool the credulous. The second is to prepare the reader for Gullivers eventual escape. This happens in his traveling box which is then conveniently destroyed by the sailors who rescue him so that no substantial evidence of his adventure remains, and the gullible can easily believe the whole story of Gulliver among the Brobdingnags. The comic irony is an effective device in satirizing human folly. The absurdity in the relationship between these two elements is essentially targeted at England (Gulliver), the Wigs, specifically, whereby Swift is attacking his opposition. In the spirit of Swifts famous word play about his good Master Bates, we can rename his fable Gullibles Travels.

(1980) studied the effects of social network interaction on life satisfaction in older adults. Research exploring the life satisfaction felt by retired physicians showed that greater optimism resulted in a greater satisfaction with their life. (Austrom, Perkins, Damush, and Hendrie, 2003) In retired individuals, especially, optimism may be an essential variable for achieving life satisfaction as it may also be a coping mechanism to the sudden change in lifestyle for the said individuals. The retired physicians felt that the greatest challenge going against their satisfaction with life was in the loss of their professional roles, thus, optimism might have served as a form of mediation between the two stages of the transition. Conscientiousness has been investigated by many researchers in terms of how well it predicts an individualââ¬â¢s life satisfaction. This trait refers to an individualââ¬â¢s tendency to be organized, diligent and reliable in their behavior. (Chapman, Duberstein, and Lyness, 2007) Conscientiousness may have a role to play in satisfaction because conscientious individuals are able to have more mature defenses and are also able to have a quality of life that is considered by most to be above par as they are able to have more responsibility and control over their health, their social interactions, and their general well-being (Chapman et al, 2007). It may well be that the same link can be found between conscientiousness and life satisfaction. If higher levels of conscientiousness indicate higher quality of life, it may also indicate greater degree of satisfaction with life as a result of the same mechanisms. à à à à à à à à à à à A study by Lounsbury, Saudarga, Gibson, and Leong (2005) examined just this relationship. Through an inspection of the personality characteristics accounted for in the Big Five, it was found that conscientiousness along with extroversion, agreeableness, neuroticism, and openness to experience account for 45% of total perceived life satisfaction. Is conscientiousness, then, as a variable independent of the other personality traits in the Big Five, significantly related to satisfaction with life? à à à à à à à à à à à The present study aims to investigate the relationships that exist between satisfaction with life and the three variables stated above: social network size, optimism, and conscientiousness. Based on the empirical evidence provided by past literature, it is predicted that satisfaction with life will be significantly correlated to optimism and conscientiousness. A greater level of optimism and conscientiousness in an individual will indicate a greater satisfaction with life. Also, social network size is hypothesized to have no significant difference on satisfaction with life. The last hypothesis is based on the findings of past literature. However, due to the questionable nature of past studies and how these measured social network against life satisfaction, the present studyââ¬â¢s hypothesis may turn out to be negated. It is hypothesized, then, that the variables of optimism and conscientiousness will have a significant and direct relationship with satisfaction with life while that of social network size will have no significant difference on satisfaction with life. Methodology Participants à à à à à à à à à à à The participants of the study totaled 91 students, 23 (25.3%) of whom were male and 68(74.7%) of whom were female. (See Table 1 in Appendix for tabulated figures) All the participants were enrolled in Research Method in Psychology classes at the California State University, Fullerton. The ethnicity break down of the participants is the following: African American ââ¬â 1.1%, Asian (Pacific Islander) ââ¬â 3.3%, Caucasian ââ¬â 49.5%, Hispanic ââ¬â 27.5%, Middle Eastern ââ¬â 2.2%, Southeast Asian ââ¬â 2.2%, multiethnic ââ¬â 11%. 3.3% of the participants reported to having other types of ethnicity.à (See Table 2 in Appendix for tabulated figures) The range in ages of the participants was from 19 years to 46 years. The mean age was 23 years old. (See Table 3 in Appendix for tabulated figures) None of the participants received incentive for their participation. There were no extra credits or monetary compensations given in exchange for their contribution to the study. Materials or Measures à à à à à à à à à à à Positive and Negative Affect Schedule (PANAS; Watson, Clark, Tellegen, 1988). The PANAS measures trait negative affect or the affective well being of the participants. It is composed of a 10-item scale designed to measure typical experiences of negative affect. Participants are able to rate the extent to which they experience certain mood states such as distressed, upset, scared, and irritable. They are able to do this through the indicators of a 5-point scale (very slightly or not at all, a little, moderately, quite a bit, extremely). The participants were asked to indicate to what extent they felt each feeling or emotion listed during the past two weeks from the time of the survey. à à à à à à à à à à à Satisfaction with Life Scale (SWLS; Diener, Emmons, Larsen Griffin, 1985) The SWLS is a global measure for subjective well-being and life satisfaction. Diener et al (1985) defined life satisfaction as a conscious cognitive judgment life. This entails an individualââ¬â¢s comparison of their own life experiences with a self-set standard. The scale is composed of 5 items and utilizes a 7-point Likert-type scale (1-strongly disagree to 7-strongly agree). à The items of the test included statements such as ââ¬Å"The conditions of my life are excellentâ⬠and ââ¬Å"If I could live my life over, I would change almost nothing.â⬠Possible total scores range from 5 to 35. A resulting score ranging from 5 to 19 signifies dissatisfaction while scores between 21-31 signify satisfaction. à à à à à à à à à à à Life Orientation Test (LOT-R; Scheier, Carver, Bridges, 1994) The LOT-R measures generalized optimism. The test is made up of 10 items. Participants will indicate the extent to which they agree with the 10 statements in the test through a 5-point Likert-type scale (0-strongly disagree to 4-strongly agree). The statements involved sentiments like ââ¬Å"in uncertain times, I usually expect the bestâ⬠. A participant can achieve a score from 0 to 24 with a higher score indicating greater levels of optimism. à à à à à à à à à à à Big Five Inventory (BFI; John, Donahue, Kentle, 1994) The BFI was used to assess the personalities of the participants with regards to the five aspects included in the big five namely extraversion, agreeableness, conscientiousness, neuroticism, and openness to experience. The BFI consists of 44 items that ask the participants to rank themselves on a 5-point Likert-type scale (1-disagree strongly to 5-agree strongly). The 44 items deal with different types of behavior related to the Big Five. For the present study, the BFI will be used to measure the variable of conscientiousness. BFI items related to conscientiousness included ââ¬Å"perseveres until the task is finishedâ⬠, ââ¬Å"is a reliable workerâ⬠, and ââ¬Å"does things efficientlyâ⬠. Lubben Social Network Scale (LSNS-6; Lubben Gironda, 2003) The LSNS-6 is a test of a set of questions establishing ties with relatives and ties with non-relatives. Examples of these questions include ââ¬Å"How many relatives do you see or hear from at least once a month?â⬠and ââ¬Å"How many friends do you see or hear from at least once a month? The participant chooses one of the options available for each question. These answers have corresponding points. Total scores of the participants may range from 0 to 30. A higher score indicates a higher level of social network. For this research, the LSNS-6 was used to measure social network size. A high score in the LSNS-6 was taken to signify a larger social network size while a low score meant a smaller social network size. Rosenberg Self-Esteem Scale (RSES; Rosenberg, 1989) The RSES measures global self-esteem. The test is composed of 10 items. Responses are given on a 5-point Likert-type scale (1-strongly disagree to 5-strongly agree). A sample question is At times I feel like I am no good at all. Subjective Happiness Scale (SHS; Lyubominsky, and Lepper , 1999) The SHS measures global subjective happiness. The test consists of four items. Responses are given on a 7-point Likert-type scale (1-7). A sample question is ââ¬Å"Some people are generally very happy. They enjoy life regardless of what is going on, getting the most out of everything. To what extent does this characterization describe you?â⬠Gratitude Questionnaire (GQ; McCullough, Emmons, and Tsang, 2002) The gratitude questionnaire is a self-report test measuring global gratitude. The test consists of six items. Responses are given on a 7-point Likert-type scale (1-stronlgy disagree to 7-strongly agree). A sample item is ââ¬Å"If I had to list everything that I was grateful for, it would be a very long list.â⬠Tendency to Forgive Scale (TTF; Brown, 2002) The TTF is a test measuring global forgiveness tendencies. The test consists of four items. Responses are given on a 7-point Likert-type scale (1-strongly disagree to 7-strongly agree). A sample item is ââ¬Å"I tend to get over it quickly when someone hurts my feelings.â⬠Procedure Questionnaires were handed out to all participants in their respective classrooms of Research Method in Psychology at the California State University, Fullerton. Participants were given instructions as a group and were told that participation in this study would be anonymous. It was also stated that they may voluntarily choose to participate and could withdraw at anytime. The whole session took about 15-30 minutes. Participants were provided informed consent prior to the administration of the test and were debriefed after they finished. Results The results showed that individualsââ¬â¢ with a smaller social network size (mean=4.10) were significantly less satisfied compared to those with life larger social network size (mean=5.05; t(89)= -3.79, p.001). There was a noted positive correlation between optimism and satisfaction with life (r = 0.543, p = 0.01). A positive correlation was also found between conscientiousness and satisfaction with life (r = 0.222, p = 0.05) Discussion à à à à à à à à à à à The main purpose of this research was to establish whether a relationship existed between satisfaction with life and optimism, and satisfaction with life and conscientiousness. Another purpose was to establish whether social network size made a difference to satisfaction with life. The original hypothesis of the study stated that a significant positive relationship would be found between satisfaction with life and optimism as well as between satisfaction with life and conscientiousness. It was also hypothesized that social network size would not have a significant difference on satisfaction with life. The hypotheses of the present study were based on the findings of past researches. (Quinn et al, 1980; Bowling et al, 1991; Austrom et al, 2003; Chang and Sanna, 2003; Chapman et al, 2007; Lounsbury et al, 2005) The results of the current study show that there is a significantly positive relationship between satisfaction with life and two variables it was compared against, namely, optimism and conscientiousness. Results also showed a significant difference with social network size and satisfaction with life. The initial hypotheses for optimism and conscientiousness were supported. The hypothesis regarding social network size, however, was rejected by the statistical results. The findings on optimism and conscientiousness validate past research findings. These showed that greater optimism in life contributed to greater satisfaction with life (Austrom et al., 2003). Optimism was also found to be a good predictor of life satisfaction (Chang and Sanna, 2003). Past findings established conscientiousness to be a contributing factor to life satisfaction (Lounsbury et al, 2005) as well as a variable directly related to higher quality of life ratings (Chapman et al., 2007). The findings on social network size, on the other hand, disagree with past research findings where social network interaction was not found to be related to life satisfaction (Quinn et al., 1980) and where the size of the individualââ¬â¢s social network was determined to be a bad predictor of life satisfaction (Bowling et al., 1991). Optimism may be able to affect life satisfaction positively due to the fact that a positive outlook on life can also cause a better assessment of past experiences not just of present circumstances. If one is able to achieve a better disposition towards life, the tendency to overlook the negativity that will detract from satisfaction felt towards life will be greater. This shows that the statistical significance of optimism (r=0.543, p=0.01) with satisfaction with life is warranted. Conscientiousness, on the other hand, was also positively correlated to (r=0.035, p=0.05) with satisfaction with life. This may be due to the fact that conscientiousness indicates a better ability to handle life experiences. Conscientiousness, as defined in the Big Five Inventory (Donahue et al., 2001), entails caution, dependability, organization and responsibility. These characteristics when applied to the everyday behavior and experiences of an individual are most likely to indicate an individual who achieves success. People who are more cautious, more dependable, more organized, and more responsible are the ones who are achievers in human society. It may be that the success and achievement linked with conscientious people is also the link that sustains their satisfaction with life. This is not to say that individuals deemed to have low conscientiousness are not likely to feel satisfaction with life. The findings only suggest that a high level of conscientiousness predicts life satisfaction to great extent. The discussion of how social network size is related to satisfaction with life should be done with care. The fact that previous research found no significant difference between social network and life satisfaction may have been due to the inadequacy of measurement with the past research. Quinn et al. (1980), for example, concentrated on the interaction that occurred in social network and not size. This meant that Quinn et al. (1980) focused on the quality of the individualââ¬â¢s social network and not on the quantity. Bowling et al. (1991), on the other hand, compared two different localities and this is what might have caused the inconsistencies in their findings concerning social network size and life satisfaction. Inherent characteristics of urban and rural locations could have played into action and caused the insignificant findings. For the present study, however, the significant difference between social network size and life satisfaction makes sense especially because social network size is also an indicator of an individualââ¬â¢s degree of social interaction as well as sources of social support; both of which are essential in an individualââ¬â¢s development. The findings of this study are limited because of the small sample size used. A bigger sample that is more representative of the general population should be used in future research. In addition, only a few variables concerning satisfaction with life were investigated. Future research should incorporate more variables that may affect life satisfaction into the study. The variables of social support, social interaction, and pessimism are a few of the factors that should be investigated. The significant relationship between social network size and life satisfaction should also be validated by future studies as the results in this study are not in agreement with previous works. The implications of the study are far-reaching. Establishing the relationships existing between life satisfaction, optimism, and conscientiousness allows different clinicians and practitioners in the healthcare system a chance to improve their handling of clients with low satisfaction with life. This may most likely involve older adults. Because satisfaction with life in itself is a concept that health-care workers find hard to deal with, finding other personality traits and variables that are related to it enables these workers an alternative in aiding these types of patients. Increasing optimism and improving conscientiousness in an individual can help to increase their satisfaction with life. In addition, increasing the size of the clientââ¬â¢s social network will improve their satisfaction with life. The present studyââ¬â¢s findings can also be expanded to teachers in the field of education. Satisfaction of their students can be increased by allowing them to feel more optimistic about their activities also by guiding them to be more conscientious in their behavior. Perceptions And Experiences Of Survivors Of Retrenchment Philosophy Essay The preceding chapter justified the selected methodology as an appropriate research strategy in the framework created by the research problem and literature review. This chapter presents the findings obtained from the present study. It represents a thematic synopsis of the prevailing survivor responses obtained through three in-depth semi-structured interviews, where each core theme is described followed by a description of the contributory sub-themes within the core themes as well as aggravating factors (if applicable). Themes will be illustrated with direct quotations in a manner that adequately addresses the construed experiences of the interviewed survivors. The first core theme to emerge is the prevailing end state of survivors in the present study. The survivors demonstrated differing forms of denial, and these manifestations are illustrated as the subsequent three core themes, with their aggravating factors. The next core themes to emerge relates to the affective and behavioural responses of survivors throughout the retrenchment process. Finally, the theme underlying and giving rise to the other themes is that of the survivors perception of the company retrenchment experience. The chapter concludes with a critical discussion of the present study in light of previous research and literature in the field, and guidelines for organisations that are contemplating, or have already commenced, with retrenchments to reduce the anticipated negative effects of the retrenchment process. 5.2 RESEARCH RESULTS AND FINDINGS In this section, the themes obtained from the gathered data are considered. As previously noted, the interviewees were quoted verbatim these quotes are demarcated by the use of blue, italic text type. Journal and memo inclusions are demarcated by the use of italic text in shaded boxes. In order to illustrate the perceptions and experiences of survivors of retrenchment, the outcomes of the present study are represented in Figure 5.1. 5.2.1 Denial The first theme elicited from the interview transcriptions is that of survivor denial. Other researchers (De Vries Balazs, 1997; Greenhalgh Jick, 1989; Noer, 1993) have explored diverse defensive responses provoked by the retrenchment experience. One frequent and widespread response is denial, a defensive mechanism equally manifesting in both management and employees who is currently experiencing, or have recently experienced, retrenchment. Denial can be defined as unconscious negation of some or all of the total available meanings of an event to alleviate anxiety or other unpleasant condition. Denial covers situations in which individuals in words, act, or fantasies attempt to avoid painful reality. (Dorpat, 1983). Denial assisted the survivors in regulating their feelings of grief, thus protecting them from anxiety and pain by refusing to completely experience happenings that the survivor could not cope with. In this regard, there is a charm in denial, as it only lets in as much grief as can be handled. However, the survivors could not work through these initial stages of the grief cycle, and subsequently, became stuck in the denial phase. This outcome is the result of various contributing factors, which has been identified as themes supporting this end state. All three interviewed survivors were in a state of denial, as they had not developed or were not equipped with constructive coping mechanisms to deal with the stress and anxiety caused by the retrenchment process. According to Chapman (2009), individuals reactions to distressing events are as unique as a fingerprint. This is clearly demonstrated as the denial manifested in three forms, which will subsequently be discussed. Fish bowl reaction The first manifestation of denial was that of the Fishbowl effect this is characterised by the survivors experiencing a pervasive sensation of being under constant scrutiny from other employees affected employees in the organisation. The survivors are scrutinised as a result of their position within the company whether it be hierarchical position or divisional position. Although the mental process of denial cannot be observed directly, it can be inferred from verbal behaviour. To this effect, Cramer (1987) developed categories for identifying denial. Pertinent to the fish bowl effect are: Statements of negation: An individual contradicts an action, wish or intention, which, if acknowledged, would cause discontent, pain, anxiety or humiliation. Again, you know, and then it seems almost like the fairy tale situation where I am ecstatically happy you know, you get your days where you just want to dart out of here and it is too much Overly maximising the positive or minimizing the negative: Sizeable exaggeration or underestimation of a characters quality, size, power, role and authority. I was involved, sitting in at the meetings with some of the directors and general managerswe did discuss some of the positions that might fall awayI was involvedbut not the actual decision making. Survivors soon realise that working in this fishbowl, everyone scrutinizes what they are doing and because other employees can offer their critique, looking good becomes as important as doing the right thing. It appears that initially, the interviewees are intensely aware of the scrutiny and their role in the unpleasant exercise, however, in order to escape the anxiety that this creates; selected stimuli and observations are unconsciously denied. You are emotional when you in our function you cant be emotional, you need to kind of your emotions need to be put to the side to stay professional. So we were stretched, and I was stretched beyond that I thought that I could do or endure. But luckily we went through it. Further to this, in the fish bowl effect manifestation, considerable attention was given to the details of the issue as this served as a distraction enabling the survivors to avoid the affective whole. This was clearly demonstrated by one survivor who insisted that the company did not communicate the financial specifics for the severance package. However, when I reviewed the company documentation, this seemed to be the one element that was clearly and consistently communicated. Kets de Vries and Balazs (1996, 1997) reported similar findings. I would have handled the process in such a way that employees knew exactly what the package entailedknow where you are at lets have a look at maybe, how does your Provident Fund look like. Keep more information, so that people can make a more informed decision. In the present study, denial appeared to be an extension of the initial avoidance response. When reality threatened the survivors directly, and when the behaviours of avoidance were no longer adequate, the survivors turned to denial. The difference between denial and avoidance is the difference between passivity and activity, or between the tacit and the overt (Gottlieb, 2004). however, the meeting was postponed on the first occasion, due to the participant being too busy on the specific day. The interview was re-scheduled on this day, I had to remind the participant of the interview, yet the interview started 10 minutes late. I found it rather disturbing that the participant was rather non-committal with some of the answers, not committing to any specific viewpoint, but rather skirting the issue as to not tick anyone, thereby remaining on the fence. The participant seemed to take on the role of champion of the company, even though most answers pointed towards disagreement. During the interview, she spoke freely about relevant retrenchment issues, but it left me with a feeling of superficiality, as the issues raised were addressed fleetingly (this seems to be a denial in itself?) and the topic was consistently changed after a few minutes, almost contradicting what she said earlier. The change of topic occurred when discussions about emotions were further explored. It seems that the recall of intense emotional experience (negative) during the period of retrenchment creates feeling of anxiety that is denied and suppressed. Non-committal answers might be an attempt to deny negative experiences and or defend the company out of a sense of loyalty to organisation or defending own role in retrenchment. In the fishbowl state, the survivor is so preoccupied with doing the right thing and looking good to both management and employees that they consistently deny and bury their own emotional responses to retrenchment. This manifestation of this denial encompasses the alteration of language and logic to justify their behaviour (Cramer, 1987; Stein, 1997). Looking back on the entire process and where we are now, a year later you think it was a good business decision? At the time yes, for the business to continue, because we didnt know the economy was going to turn. Yes, I would say at that given point in time it could have been a good business decision. all affected people kind of made good business senseà ¢Ã¢â ¬Ã ¦I dont know if that makes any sense at all but made good business sense. So I think everybody would understand that none of this was a personal mission to get rid of anybody. It is possible that, as a consequence of prolonged denial in the fishbowl state, the survivors reasonable argument and cognitive schemata might be discarded altogether, because those strategies are not sustainable and are unable to persuade others; at which time the survivor will merely refer to their feelings or emotions as the exclusive justification. (Cramer, 1987). Placing all your eggs in the downsizing organisations basket It would seem that the strategies of retaining employees for the long-term have had negative, inadvertent consequences on the survivors seducing employees into a co-dependent relationship with the organisation. The co-dependent employees sense of value and identity is based on pleasing someone or something else. A lot of duality in this interview unhappy at work, yet wishing to stay, also expressing regret at not being retrenched, yet sacrificing her relationship to perform responsibilities at work. Defending abuse from management, yet complaining about management style. According to Noer (2009), benefits, services and office size are all advantages that reward seniority and tenure. In addition, Company X has in recent years placed more focus on employee wellness programmes, thereby channelling employees social patterns into organisationally endorsed activities. The result is that numerous employees have placed all of their emotional and social eggs in the organisational basket as the retrenchments were implemented and the new psychological contract unfolded, the basket has been dropped, and, according to Noer (1993), resulting in a range of negative survivor symptoms, such as fear and anxiety and triggering co-dependent behaviours like control and denial. The duality experienced could possibly be viewed as anxious denial there seems to be a swing from one extreme to the other, as if she cannot consolidate these conflicting emotions and feelings and the anxiety that this is causing. This denial and need for control is verbalised / acted out in the desire to have been retrenched. Due to the role that I fulfil on Organisation X, I have been privy to discussion about subsequent decisions made by this participant affecting / sacrificing her private life for the benefit of her career. As previously discussed, the higher the organisational level, the stronger the denial tends to be. It is therefore not surprising that the interviewee who most strongly displays this manifestation of denial, is the highest ranking employee interviewed. Although the concept of co-dependency will be discussed later in this section, it should be noted that previous research (Noer, 1993) indicated that co-dependants make themselves into permanent victims. It seems that perhaps this is the most salient characteristic of this manifestation of denial by denying any other role other than that of the victim, the survivor has rejected their role and accountability as part of the senior management team of Organisation X, as well as rejecting activities that should have been undertaken as part of management responsibility. Thereby effectively denying the extent to which her lack of action contributed to negative impacts on others, specifically the affected subordinates. A possible explanation for this unconscious decision is that it is an attempt to avoid guilt, and possibly shame. but with even like with subordinate I worked with, right until a week which before she moved, she didnt really know what was going to happen to her. She was told, Divisional Manager said to me, well maybe she would fit in with General Manager: XXXs department. So I went back, said to Subordinate, please set up a meeting with General Manager: XXX go speak to him, see what the job is all about. But nobody spoke to her I spoke to her said, lets make an appointment with General Manager: XXX.so nobody spoke to her, it wasnt like a person or thing. I know with Subordinate, nobody came to speak to her to say you know you are going to lose your position, but there is another position for you this is what it entails or set up an appointment with General Manager: XXX or anythingI would have like to just walked with my people a little bit more, so that theyre not so unhappy. There were lots of rumours and think from that aspect, maybe they were not as open as they should have been. This is congruent to two of Cramers (1987) characteristic of denial, namely the denial of reality whereby the survivor avoids addressing something that would be unpleasant to think about, in this case the loss of her subordinate to another department as well as her abdication of responsibility as departmental head. Secondly, statements of negation where the individual contradicts an action, wish or intention, which, if acknowledged, would cause discontent, pain, anxiety or humiliation. Further to this, and supporting the previous assertion of the strong denial of management responsibility, dissatisfaction with company direction, management credibility and long-term strategy were particularly strong amongst the most senior survivor interviewed, which is interesting as she is responsible for some of these functions. A lot of the instructions that were given are very short term. So, I think from an environment point of view, I just feel Im not learning as much as I could have, because everything is now killing the fires and its short term strategies, so you dont actually learn or develop and there is not time to develop as a person within the company. How would you describe your level of trust, in the management team of the company?very low trust. Because if something goes wrong, they blame somebody else. This candidate is part of senior management, yet seems to feel ostracized. Perhaps this is the reason for her projections? Further to the above, when psychological denial distorts reality, individuals are prone to make erroneous decisions or avoid complicated decisions and disregard serious problems; holding others responsible when things do not turn out as expected (Cramer, 1987). This finding is supported by subsequent verifiable information that became known to me recently as a result of the interview and my position within the organisation. one interviewee fluctuated between almost total denial of her accountability and role (as senior manager) in the retrenchment and repeated attempts to bring about her own death (Kà ¼bler-Ross, 1969) in the organisation. This self-sabotage took the form of non-performance, defiance when dealing with top management and ignoring communications. Some aggravating factors to this denial reaction must also be noted the first is the espoused value of the new psychological contract and secondly the presence of aggravating peripheral circumstances. These factors will be explored and discussed later in this section. Keeping up appearances: coping by not coping This manifestation is characterised by the minimisation of the overall effect that the retrenchment had on the survivors. Although feelings of uncertainty, stress and guilt had been experienced, there were some expressions of optimism and perceptions that Company X engaged in a tough, but necessary activity to get or keep the company on track toward profitability. I think for anybody this is a very difficult exercise to go throughà ¢Ã¢â ¬Ã ¦ there was not you dont need to worry, it is never going to happen, youre safe. There was never any of that false hope created. I dont know if that makes any sense at all but made good business sense. Unfortunately, as a business, this is the only way we can survive. This technique has been termed denial of injuries by Gandolfi (2009) and pertains to survivors involved in the retrenchment process maintaining that retrenchment victims did not suffer any detrimental consequences. In the aforementioned study, this was the reaction where victims were provided with generous severance packages. The findings of the current study in consistent with this, as all interviewed survivors exhibited a manifestation of denial and without fail, referred to the generous severance packages. Well, I looked at the packages and I know that they were really good based on obviously you know what the packages based on experience, based on years service and based on beginner service and all those sort of things. However, the denial is evident in the contradictions during the interviews, as well as during discussion that pertained to behavioural and affective responses during the retrenchment. This is congruent with two of Cramer (1987) identified characteristics of denial, namely: Statements of negation: An individual contradicts an action, wish or intention, which, if acknowledged, would cause discontent, pain, anxiety or humiliation. Obviously I could not divulge that until the final decisions had been madeà ¢Ã¢â ¬Ã ¦ when you are going through a structural change there is this cloud of secrecy and certain people know whats going on, and there is talking à ¢Ã¢â ¬Ã ¦it was communicated to them [affected department], and it was done right down to the brutal truth. Unexpected goodness, optimism, positivity: nonchalance in the face of threats. The team is not quite where it needs to be yet, but certainly we pulling more together in terms of the transparencies and the cross functional support areas where we may be 5 years ago. I got to say the resizing exercise did a lot in terms of shaking up peoples foundations where you thought you were so secure, everybody realized nobody is secure. But what changed it and what made this last resizing exercise so positive, is the way it was handled. These reported perceptions are complex and quite often contradictory. The same survivors who articulated sentiments of optimism also articulated strong sentiments of uncertainty, stress and changed work life perceptions. Uncertainty was most apparent in the continuous attempts to obtain the approval and validation of reactions from me, as if to establish whether the perceptions are correct, or perhaps to avoid confronting actions that causes guilt by perceiving this to be a common occurrence. It seems as if the survivors escaped dealing with their personal feelings by focusing on projected organisational outcomes. This is congruent to previous research (Noer, 1993) as well a variation on the fishbowl effect, where survivors focus on details to escape dealing with their personal feelings. The objectives were pretty clear and that was that we need to look at the functions within the department, and the main objective was not clearing heads, that was never the main objective. The main objective was look at the functions, how can we work and structure this department to work smarter. Not necessarily cheaper, but smarter and obviously ultimately is there a possibility for the right sizingthe objective was never to get the heads of the headcount, never. Obviously you know long term wise it is a win- win situation for everybody. This particular manifestation seems to be exacerbated by previous experience of a retrenchment possibly colouring the survivors frame of reference. The interviewee that displayed this manifestation most prominently has personally survived two previous retrenchments at Organisation X. Another interviewee who displayed this manifestation of denial to a lesser extent had survived one previous retrenchment at Organisation X and recalls that her father had experienced a retrenchment at his employer at the time. I am left to wonder whether this positive attitude is genuine or whether the participant is still keeping up appearances for the benefit of who perhaps top management perhaps. Is this denial (of damage done to others) a facade to hide feelings of being the executioner as the participant clearly indicated that she felt guilty for being picked to stay whilst other employees (in her department) became victims. This reaction can possibly be attributed to repeated experiences of retrenchment resulting in emotional numbing (Kets de Vries Balazs, 1996), whereby the survivor starts to experience problems with a reduced capacity to feel. According to Kets de Vries and Balazs (1996), this defensive process is set in motion a state of affairs overflowing with painful emotions and psychological conflict. This reaction was used as a means to shelter the survivors from intolerable experiences and functions as a type off shut-off mechanism this emotional shut-off is essentially the core of psychological denial as discussed in the introduction to this section. It appears as if the survivors resort to doing to prevent experiencing. One of the survivors went into great detail about her growth in the company, her flexibility with regards to long-term career goals, the positive relationships with both top management and Human Resources, and her perception of how other departments handled this process. In all of this, she never voluntarily referred to feelings. When specifically asked about her feelings and reactions during the time of retrenchments, she conceded that she did feel saddened by the process, but quickly added that this was reserved for after-hours and in general, she felt that it had been a process well handled. What seems to be true for all of the participants is that their involvement in retrenchments is an activity that opposes their base belief about organisational life. Underlying this base belief is the psychological employment contract. These survivors have to cope with significant change at the same time as experiencing it. Shared denial Although the survivors engaged in the same basic defence mechanism, the expression of the defence took various forms. It is interesting to note that there are some shared reactions in the expression of denial, namely the denial of the survivors own expectation of the old psychological contract to hold true and the unfailing blaming of others for negative outcomes experienced. Each of these shared reactions will consequently be explored. Denial of belief in old psychological contract All of the interviewees espoused the values of the new psychological contract and advocated its importance in the new world of work, yet, all the interviewees demonstrated throughout the interview, that they have a very strong expectation that the old psychological contract is to hold true for them on Organisation X. . I really would like to have an overseas assignment. Just to get exposure, because for me is I want a long term relationship with Company X. I was quite happy here and I thought, well, I am happy to stay with Company X for the next 10 years. Whatever I wanted to develop, I would like to get to a General Manager level, learn as much as I can from the company. It seems that the survivors agree with the logic and the theory of the new psychological contract, but that this new way paradigm conflicts with their need for membership and would require a great mindset shift. I believe that most employees expect an organisation to look after them and that you will receive the benefits, or reap the benefits of hard work. But in reality, that does not work that wayI know that your psychological contract has changed during the course of the last few years, but still, I think that maybe employees want to be trained in such way that they can move on, out of the organisation, with more skills they what they come into. Despite strongly advocating the new psychological contract the participants responses throughout the interview indicates a strong co-dependency on the company being researched. It seems as if the participant defines a (big?) part of herself by achievement / growth / future in / at the company being researched. Also, this participant perhaps more than the other participants espouses the values of the new psychological contract, yet looks at the organisation to provide growth and development, and feels betrayed and offended that the current situation impacted on her development. The survivors denial of their belief in the old psychological contract could possibly be explained by the concept of co-dependence. The co-dependent changed their identity, denied their feelings and spend a substantial quantity of energy in an effort to control an alcoholic; shared the alcoholics addiction. The co-dependent does not notice the destruction that their denial causes to themselves and others they were co-dependent with the alcoholic (Bekker, 1998; Noer, 1993) According to Noer (1993), just as a person can exist in a co-dependent state with another person in relation to an addiction, a person can also be co-dependent with an organisational system. Employees of an organisation who have become co-dependent on the organisation, defines themselves to a large extent by their job. During a retrenchment, it is therefore not only their job that becomes vulnerable, but also their sense of worth, sense of relevance, identity and purpose. This phenomenon could explain most, if not all, of the survivor experiences explored in this study. As discussed in section 3.3.4, the greater the sense of personal violation, the greater the susceptibility to survivor syndrome and destructive defence mechanisms. The perception of violation appears directly related to the degree of trust employees had that Organisation X will take care of them (Noer, 1993). The survivors all indicated that they definitely had the expectation of long-term employment at Company X, with the associated growth and development as reward for their tenure and performance. The retrenchment experience seemed to be a rude wake-up call that this is in actual fact not the truth. There is no such thing as life-long employment. There is a classic saying, if you want loyalty, buy a dog. It is all good and well to be committed to a company, but there is no guarantee, not either way, that you are going to be with that company for a life-long commitment or that that the company is going to keep you on for a life-long commitmentonce you heard that very clear definition that life owes you nothing and Company X owes you nothing and I owe you nothing. That kind of inspired you to do more and to want more. You know, if I came in at age 17, turning 18 in February, that I was working, thinking I am just going to be a typist for the rest of my life and Company X going to keep me, keep me, you gonna have to want that little bit more ever so often. If you are not motivated, and if you are not accepting those challenges, there is no future for you here. Denying their belief in the truth of the old psychological contract, might serve two purposes: By verbally acknowledging the value of the new psychological contract, they are espousing the message that is expected of them due to their position within the company. As in the fishbowl effect manifestation, the survivors are seen to be doing the right thing and looking good; By denying their belief in the truth of the old psychological contract, the survivors are attempting to avoid the painful reality that this paradigm no longer holds true by refuting their own truth by their words and actions. Blaming of others All interviewed survivors blamed other groups. In most cases, generic management were blamed or colleagues in organisation X, however, no interviewee assigned any blame for negative outcomes to themselves. Nevertheless, people tend to blame others usually the next person up on the organisational chart for what is a basic systemic change, beyond anyones control. But then, if I look at other areas that the same sort of exercises, and I dont know if it is because it was handled poorly, that maybe their management wasnt being honest or forthcoming with their people. Look at the XXX department a huge mess. You know, where people left on the end of the month, and there was absolutely nobody trained to do those. If youre ultimate goal is to get rid of people on downsize and get rid of people on a head count level that is easy to do. This blaming phenomenon could be a form of projection that serves as a defence mechanism, assisting the survivor to confront their own survivor guilt by allowing the survivor to consider others as dysfunctional without experiencing the discomfort of realising that these views and feelings are their own. This allows the survivor to express disapproval of the other person, distancing themselves from their own dysfunction (Straker, 2009). This reaction has also been noted by previous researchers such as Noer (1993), Kets de Vries and Balazs (1996) and Appelbaum et al. (1997). Based on the preceding discussion, it is put forward that blaming others for whichever collection of consequences generally represents denial. Contributory themes As a result of the survivors inability to progress through the stages of grief and complete their mourning, the interviewed survivors remained in state of denial. Contributing to these responses and outcomes were the presence of aggravating peripheral circumstances and role conflict. Role conflict Beauchamp and Bray (2001) defines role conflict as referring to the presence of incongruent expectations placed on a role incumbent. The presence of role conflict exacerbating the effects of denial was evident amongst survivors. This role conflict related to the dual roles of being involved in the implementation of the retrenchment for the survival of Company X and the emotional effect that the aforementioned has on the survivor. You are emotional when you in our function you cant be emotional, you need to kind of your emotions need to be put to the side to stay professional. So, you talk to people over the telephone, and they are emotional that side, youre sitting on this side, and you know their affected it is a difficult, difficult time. It is possible that this role conflict also contributed to the extensive engagement in reasoning to justify retrenchments. This reaction is similar to what Noer (1993) described as the Judas complex. This reaction seems to allow the survivors to deny the negative impact of their actions on others. It is very difficult to keep your managers hat on and on the other side of the scale to put your humanitarian hat on.you had to come in

Why Psi: Perceptions of Anomalous Cognition Why would I risk the ridicule of my peers to explore a topic as controversial and inflammatory as that which is known as "psi" phenomenon? And more importantly how and why is it relevant to the study of neurobiology? Given the nature of the topic, it may not always be easy to discuss such scientifically or diplomatically. It is my contention; however, that serious review of the scientific evidence, legitimate scientific discourse and funding for research has been seriously impeded because of its controversial nature. And, furthermore, that it is as relevant a topic for investigation as are other methods by which information is perceived by humans. There are some valid reasons for the prejudice that surrounds this topic. Often when one thinks of psi phenomenon some strange themes come to mind such as UFO's and Alien Abductions, ESP, crop circles, Astrology, Ghosts, Mediums, Channeling and Angles. Most of these come under the heading of "paranormal" and are decidedly difficult to examine scientifically. The plethora of information on such subjects that floods the internet is more likely to reference mysticism, faith and spiritually than offer scientific theory or experimental outcomes and I will leave such to those realms. I, therefore, in this paper will not be referring to alien astrologers channeling their horoscopes through mediums who have been abducted. Only the concepts that comprise that which is known as "anomalous cognition will be discussed." Its components are telepathy, clairvoyance, and precognition (1). My first exposure to the term "anomalous cognition" came as a result of my research for this paper. I just had always called it "knowing." Ever since I was a little girl, however, I have been repeatedly told that my experiences were "all in my head." That had been exactly my position also. But what my critics, who included my family members, friends and my husband, really meant was that my experiences existed only in my imagination. They argued, as is commonly believed and sometimes verified, that experiences like mine could be caused by selective or faulty memory, wishful thinking or subliminal clues (2). And for many years, I tried to believe that one of those explanations applied to me. My husband the scientist, engineer and amateur magician has been telling me for years what he believed to be the prevailing opinion about psi within much of the scientific community. The picture was not very flattering.

Contemporary liberal and anarchist philosophy are both two very different ways of trying to see what would be the best way to run society. While discussing these two ideologies I will try to show how both, in their purist sense, are not able work in today's society effectively. Contemporary liberals are involved in every day politics but through over regulation and dependence on government they loose their chances of running a reliable democracy. Anarchist have very good ideas of how a natural society could function without government or modern institutions but the biggest problem they have is how to get to that point. Both theories look good on paper but once they hit the real world they change due to alternating conceptions and individual influences. The root of the word anarchism comes from the Greek word anarchos, which means without ruler. The main philosophy behind anarchism is that people can reside in an unregulated community with no real authority and maintain a sustainable life. Anarchists see government and capitalism as an institution that creates liberty for the rich and enslavement of the masses. Emma Goldman best describes anarchism as: The philosophy of a new social order based on liberty unrestricted by man-made law; the theory that all forms of government rest on violence and are therefore wrong and harmful, as well as unnecessary. With anarchism there is a belief that once all government is abolished by the people that everyone will come together in a community of mutual aid and understanding without laws or authority to direct. Their philosophy can be considered opposite of most other ideologies, especially that of contemporary liberalism. Contemporary liberalism strives to hold on to the classic liberal's ideals pertaining to political, economic, and social liberties but it tends to look at democratic government as a tool rather than a hindrance. John Stuart Mill, John Dewey, and Franklin D. Roosevelt are established ideologues of contemporary liberalism. Just opposite of anarchism, modern liberalism puts its' faith in government to change and adapt to the failures of capitalism. The emergence of this political philosophy started around the end of the nineteenth century with John Stuart Mill's ideas in his book Principles of Political Economy. I would love to live in a society where you just did what was good for you and helped others while they helped you. I don't know why anyone wouldn't want to live in a society like that. The only problem is that to get to this utopian society the whole world would have to spontaneously decide they wanted it and I can guarantee there are many politicians and CEOs that would not like that. Contemporary liberals are right in wanting to reform institutions of today but I can't see how over regulation of government in all areas is going to help fix things. I do see the need for some government action towards the eradication of poverty, equal rights, and unemployment. Epidural Epidemic Drugs in Labor: Are They Really Necessary. . . or Even Safe? The use of epidurals is so common today that many perinatal professionals are calling the 1990s the age of the epidural epidemic. Believed by many in the medical profession to be safe and effective, the epidural seems now to be regarded as a veritable panacea for dealing with the pain of childbirth. It is true that most women experience pain during the course of labor. This pain can be intense and very real, even for those who have prepared for it. But pain is only one of many possible sensations and experiences that characterize the experience of giving birth. Barbara Katz Rothman, a sociologist who studies birth in America, writes that in the medical management of childbirth, the experience of the mother is viewed by physicians as pain: pain experienced and pain to be avoided.1 Having experienced childbirth ourselves, we have great compassion for women in painful labors. However, we also feel a responsibility to mothers and their babies to explore issues concerning the use of epidu ral anesthesia in labor issues that are seldom discussed prenatally. Several factors make the use of epidurals potentially hazardous. The Physician’s Desk Reference cautions that local anesthetics – the type used in epidurals – rapidly cross the placenta. When used for epidural blocks, anesthesia can cause varying degrees of maternal, fetal, and neonatal toxicity which can result in the following side effects: hypotension, urinary retention, fecal and urinary incontinence, paralysis of lower extremities, loss of feeling in the limbs, headache, backache, septic meningitis, slowing of labor, increased need for forceps and vacuum deliveries, cranial nerve palsies, allergic reactions, respiratory depression, nausea, vomiting, and seizures.2 In addition, a piece of the catheter that delivers the drug into the duraregion of the back may break off and be left in the woman, a dangerous risk that necessitates surgical removal. One of the most well-known side effects of spinal anesthesia is a spinal headache. Depending on the amount of anesthetic used and how the catheter was placed, the headache can be mild or severe, lasting between one and ten days after the birth. This is not how any of us wants to feel in our first days and hours with our newborn. Epidurals also have been linked to an overall increase in operative deliveries: cesareans, forceps deliveries, and vacuum extractions. A meta-analysis of the effects of epidural anesthesia on the rate of cesarean deliveries was undertaken by a group of physicians who examined, categorized, and analyzed all available literature.
